石灰化を呈し抜去が困難であった腟内異物の一症例 A case of vaginal foreign body which was calcified and difficult to remove 榛葉, 賴子 SHINBA, Yoriko 矢田, 大輔 YATA, Daisuke 小田, 彩子 ODA, Ayako 東堂, 祐介 TODO, Yusuke 伊藤, 敏谷 ITO, Toshiya 鈴木, 康之 SUZUKI, Yasuyuki open access 腟内異物 石灰化 膀胱腟瘻 腟内異物が放置される例は、産婦人科ではまれに経験される。症例は50歳。スプレー缶の蓋を腟内に自己挿入し、抜去できずに約2年間放置していた。抜去目的に当院を紹介され、腰椎麻酔下に子宮把持鉗子で抜去した。抜去された異物は、石灰化物質で覆われており、成分分析では、リン酸カルシウムとリン酸マグネシウムが検出された。報告では腟内のカルシウムイオン、マグネシウムイオンは尿の成分に由来 すると結論づけられるものが多い。腟内異物の症例では膀胱腟瘻を来していることも多い。 本症例では画像検査で膀胱腟瘻などの解剖学的異常はなく、抜去可能であった。長期間放置された腟内異物の診療では、膀胱腟瘻などの解剖学的異常も含め、画像検査で評価することが有用である。 A case that foreign bodies in the vagina are left is rarely experienced at the obstetrics and gynecology department. The patient, a 50-year-old woman inserted the cover of a spray can into the vagina by herself and left it for two years. She was referred to our hospital to remove the vaginal foreign body. We could remove it by using uterine grasping forceps under spinal anesthesia. The component analysis revealed that the removed material was covered with calcium phosphate and magnesium phosphate in the present case. Many reports conclude that the calcium and magnesium ions in the vagina are derived from the components of urine. In the cases with vaginal foreign body, the patients have complications, such as vesicovaginal fistula. Fortunately, our case had no complications.
