Japonica","bibliographic_titleLang":"en"}]}]},"item_3_description_9":{"attribute_name":"抄録","attribute_value_mlt":[{"subitem_description":"母親にビタミンKを投与し母乳のビタミンK不足を補うことにより特発性乳児ビタミンK欠乏性出血症を予防できないかと考え以下の研究を行った.\n早期産褥婦60例をMenaquinone-4 (MK-4) 投与群, ビタミンK_1投与群, 対照群の3群に分け, 産褥4日目に採血と採乳を行い血漿中および母乳中のビタミンK_1, MK-4, MK-7濃度を測定し比較した.その結果, MK-4投与群ではMK-4濃度は血漿中2.13ng/ml, 母乳中49.3ng/mlであり, 対照群のMK-4濃度が血漿中0.28ng/ml, 母乳中1.51ng/mlであるのに比しいずれも有意に高濃度であった.またビタミンK_1投与群ではビタミンK_1の濃度が血漿中49.0ng/ml, 母乳中71.6ng/mlであり, 対照群の血漿中1.17ng/ml, 母乳中2.41ng/mlに比し有意に上昇していた.母乳中濃度を血漿中濃度で割り濃縮率と表現して比較すると, 対照群はビタミンK_1が2.52倍, MK-4が5.43倍, MK-7が0.52倍であり, MK-4投与群ではビタミンK_1が1.60倍, MK-4が40.2倍, MK-7は0.67倍となり, ビタミンK_1投与群ではビタミンK_1が1.65倍, MK-4は10.8倍, MK-7は0.71倍であった.ビタミンK_1とMK-4は母乳中に濃縮されているが濃縮率はMK-4の方が高く, ビタミンK_2 (MK-4) 製剤を投与するとさらに濃縮率が上昇した.それに対しビタミンK_1製剤を投与してもビタミンK_1の濃縮率に変化はなかった.MK-4投与群において母乳中濃度の経日的な変化を調べると, MK-4濃度は日を追うごとに上昇し, 産褥1日目の1.69ng/mlから4日目には49.3ng/mlとなり, 蓄積されて母乳中に移行しているため, MK-4を継続的に投与すると母乳中のビタミンK濃度をさらに上昇させ得ると考えられた.母親にMK-4を多く含む卵黄, 牛肉, 乳製品などを摂取するよう指導することや,
