薬物過敏症の診断には皮膚プリックテスト（skin prick test：SPT），皮内テスト（intradermal test：IDT）が用いられるが，文献により濃度に違いがあるなど確立されていない．症例は14歳男子，セファゾリン（CEZ）投与後に即時型過敏反応を呈した．CEZのSPT，IDTが陽性であったためCEZ過敏症と診断した．代替薬としてセフォチアム（CTM）を検討し，IDT（20mg/mL）を実施したところ陽性であった．本来CEZとCTMは交差性が無いとされ，健常者3人でもCTMのIDTが陽性であったことから偽陽性と考えた．そこで，患者にCTMの薬剤誘発試験を実施し陰性であった．IDTは薬剤により偽陽性が生じやすく，情報の少ない薬剤では健常者での確認を行い安易なIDTは避けることが重要である． |

Intradermal tests (IDT), skin prick tests (SPT), and drug provocation tests (DPT) help diagnose drug hypersensitivity, but no established method exists. A 14-year-old boy developed an immediate hypersensitivity reaction after administration of cefazolin (CEZ). Following positive SPT and IDT results for CEZ, the patient was diagnosed with CEZ hypersensitivity. Because Cefotiam (CTM) was considered as an alternative drug, and an IDT (20 mg/mL) was performed, which showed a positive result. Although CEZ and CTM do not cross-react, the IDT for CTM was also positive in three healthy controls, leading to a false-positive result in the patient. A DPT for CTM was performed on the patient, and the result was negative. IDT may cause false positives depending on drugs, and that concentrations vary depending on the literature. In infrequently used drugs, it is important to evaluate results in healthy controls and avoid hasty IDT in patients. |
