今回我々は,右自然気胸術後の切除標本で偶然発見されたmalignant solitary fibrous tumorの1例を経験した.症例は48歳男性.右自然気胸の手術は胸腔鏡下で上葉にある肺嚢胞を広範囲にわたり切除した.術前および術中は腫瘍の存在には気づかず,術後の病理診断で腫瘍の存在が明らかとなった.この患者はその後,約6ヵ月経過して胸腔内に局所再発し,放射線化学療法と手術療法を行ったが再々発し,気胸手術後約1年経過して亡くなった.solitary fibrous tumorは,再発を繰り返し時として悪性化する場合もありえるので長期間の経過観察が重要である. A 48-year old man was referred to our hospital because of right spontaneous pneumothorax. We performed bullectomy by video assisted thoracic surgery (VATS). The pathological findings revealed that a malignant solitary fibrous tumor (MSFT) consisted in part of the specimen. The disease recurred 2 times and the patient finally died about one year after the first operation. Long-term clinical follow-up is recommended for all patients with solitary fibrous tumor because of the potential adverse biological behavior of this tumor, which may lead to repeated recurrences and malignant transformation. |
